Parent Volunteers
What documents are required to volunteer?
There are 4 documents required to volunteer:
1. TB test (Mantous Skin Test or a TB risk assessment performed by a physician)
2. Fingerprinting (Live Scan) clearance (performed by the Department of Justice)
3. OPA's Privacy and Confidentiality Agreement (signed and submitted)
4. OPA's Acceptable Use of Technology Policy (signed and submitted)
How long is my TB test valid for?
Your TB test is valid for 4 years.
How long is my fingerprinting (Live Scan) valid for?
This is valid for the entire time your childl/children attend OPA.
If you have been fingerprinted as an employee, substitute or volunteer for OPA in the past, you need not be fingerprinted again for OPA.
Where do I go to get fingerprinted (Live Scan)?
There are many places within the community including many UPS stores.

If I was fingerprinted for another organization or school district, do I need to be finger printed again for OPA?
Yes. The Department of Justice prohibits organizations from sharing background information and individuals' records.
